How to Register We are currently open for new registrations for patients living in the practice catchment area. In order to register with us please request a new patient registration pack from one of our receptionists. If you have an NHS medical card, bring it along. To complete your registration you will be asked to complete the questionnaire inside the pack and return it to the surgery. We will arrange an appointment for you with a Practice Nurse so that your general health can be recorded. Your GP notes from your last Practice can take many months to arrive, so it is very useful for us if our nurse has met with you, noted your health details and made some simple measurements [e.g. blood pressure]. If you are from outside the UK, we can check whether you are eligible for NHS treatment. |
