Digital NHS Health Check - Hightown Surgery

We are offering you to complete an NHS Health check in the comfort of your own home

By making health checks more convenient and accessible, we can hopefully provide more of our eligible patients with this check and in turn identify more people who may be at risk and therefore take measures to reduce their long-term risks. Please allow 20mins for the questionnaire and answer all the questions.

If you require an up-to-date blood test you will be sent a booking link – Once you have completed the questionnaire and the necessary blood tests are done you will be given your estimated Cardiovascular risk score (your risk of developing a heart or circulation problem over the next 10 years)
