To register with the practice you can follow the link below or visit the practice and collect a paper form to complete and return.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Please note that submitting an application for registration does not guarantee you will be registered at Hightown Surgery.
